This form is designed as a safe and confidential way to report incidents against gay, lesbian, bisexual and transgender (GLBT) individuals to the Long Island GLBT Services Network. Reporting anti-GLBT incidents helps to create a safer community on Long Island.
This form is to report any bullying, harassment, discrimination, or violence against GLBT individuals or groups. You do not have to be the victim of the incident to report here. If you witnessed an incident, you can also report here. Some examples of anti-GLBT bullying, harassment, discrimination or violence include:
Someone calling you anti-GLBT derogatory terms (“faggot,” “dyke,” “tranny,” etc.);
Reporting anti-GLBT harassment or bullying to school/work staff and having no action taken against the bully/harasser.
Someone refusing you any type of service based on your real or perceived sexual orientation or gender identity.
Someone physically hurting or assaulting you while using anti-GLBT derogatory terms, or because of your real or perceived sexual orientation or gender identity (eg. They see you leaving a gay bar/club; attending a gay-straight alliance meeting, etc.)
The personal information entered on this form is CONFIDENTIAL and WILL NOT be shared with anyone else without your permission. All reports will be followed up by a staff member of the Long Island GLBT Services Network within 24 hours.
